Referral Pathway

A respectful beginning.
A plan for what comes next.

ELG is designed for young adults ages 18–24 who are experiencing homelessness, housing instability, or a need for structured transitional support.

How Support Will Work

Our planned four-step pathway.

  1. 1

    Initial Contact

A young adult or referral partner reaches out to begin the conversation.

  2. 2

    Screening Conversation

We learn about needs, eligibility, safety, and whether ELG may be a good fit.

  3. 3

    Intake & Orientation

After opening, eligible residents complete intake and learn about the program.

  4. 4

    Empowerment Plan

Together, we build a plan for immediate needs and long-term goals.

Who May Reach Out

Start a future referral conversation.

Young adults, community organizations, schools, healthcare providers, public agencies, and other helping professionals are welcome to contact ELG for program-development information.

Contacting us now does not create a waitlist placement or guarantee admission. Formal eligibility and intake procedures will begin only after certification and opening.
